    Not sure... We don't get just shot records... We have to have the official form 121 shot forms... Which say "up to date until kindergarten" or etc. here in ms, they aren't required to have the fourth dtap until they start school.

    As long as the form 121 says ey are up to date, and signed by their doctor, I'm good.

      You input what they have had so far. It then figures what shots are due and when. I give one to the parents and one in the kids files.

      But I personally think it's maddening that a daycare provider can get into trouble for what should be PARENT responsibility.

      Some providers get around it by having the parents sign an exception form.

      I'm in Utah and it's not that simple. The parent has to go to the local health dept. and talk to an official there and watch a video and then sign a special waiver form they have to bring to me to be to keep on file.

      We no longer have our immunizations checked by licensing though. We have to keep the files, but they don't inspect them.
      Each November, we have to fill out an online questionnaire that goes directly to to the immunization program. If they don't receive the online report, they then contact licensing who will issue a citation.

        My son just got his fourth dtap to start kindy.. He's five. Wile I think you CAN get them all in before they are two... It's not required until ages 4-6 years from my information too... No ideas.

        Sme immunization a here are required and some aren't. And you can't do a religious exemption for the heck of it ether unless you homeschool. They don't have to be vaccinated until school though.

            Personally, I tell my parents they can sign a waiver for all immunizations, or they must NEVER be even one day late on a shot. If they are I will meet them at the door and send them home. "Where's the form" is the answer I get.

            I'm not the parent, and I'm not interested in doing the parent's job. One more form for me to spend time. Nope, waiver for everyone.

                  I only have 6 kids, so I keep track of when their next shots are due. I stick a little post it note to my calendar one month prior to them being due.

                  For example, let's say Billy Bob turns 18 months on October 1. His shots are due sometime between October 1 and November 1 when he turns 19 months. A few days before October 1, I will say "Oh, Billy Bob is almost 18 months old! I need the date that he will be getting his immunizations. Be sure to get it done before he turns 19 months, so that I don't have to exclude him!" That reminds the parents to get him in, and my inspector will accept an appointment date as long as it is between the day the child turns 18 months and when they turn 19 months.

                  If for some reason the child will not be receiving their shot at the KDHE suggested time, I have them get a note from the doctor stating when they will be receiving the shot. My inspector will also accept this. I have had several kids that have doctors that follow a different schedule. Not sure why. My daughter's shots were always a few months late, due to health problems at birth. KDHE had no problems with this as long as I had a form from the doctor stating when the shots would be given.

                  I agree that it is a pain to have to almost force some parents to keep up with this. However, I don't want to get in trouble for it, so I do it.

                        So I went through my dck's records and my 15 mo dcg looks like she needs 3-4 by 15 months and then one by 18 months. I don't have the info in front of me so I have no clue which ones off the top of my head.

                        How would you tell the parents? What time frame? Before she turns 16 months?

                          If she is already past due and your state is as strict as mine, I would let them know immediately. Then, tell them to either get a note from the doctor stating why she hasn't gotten them yet or make her an appointment ASAP to get them. Either way, exclude until one of the requirements are met.

                          It's not worth the deficiency.
