I never lost one in my care. I have lost people in my nursing job. As far as daycare kids go. I had a little girl in the daycare center I worked at when she was 2. When she was 4 her mother locked her and her siblings in a closet and burned the house down around them 
it was terrible made me sick at the time and still does every time I think about it.
