Final Day Checklist

My licensor recommended that I have my dcf sign a form saying this was their final day. Is there anything else the letter/form should say? Any one have a sample I could see? Thank you!

I thought about creating one before, I was going to list there belongings (clothes, diapers and wipes) and have them sign saying everything listed was returned.

I use one as a means of "closure" for each client.

The first page has a place for parents to sign off that they have given proper notice, are paid in full and owe no additional fees and have collected all their child's belongings.

It also has space for them to indicate that they do or don't have any issues with my program (in other words aren't trying to report me or sue me).

The second page is client feedback in regards to whether they liked or didn't like my program and if they would use me again or not and if they would or wouldn't recommend my program to others
