    Child Care Registration & Enrollment Forms?

    Good morning,

    Yipy I'm officially (or about to be) a home daycare provider! I am excited and nervous . We recently met with a perspective family and it went splendid. They want to enroll ASAP. So the clock is ticking. I have all the registration forms provided by and mandated by the state, and a simple generic contract. However, I know I most likely would benifit from a more robust enrollment packet. In general, which forms do you all have your clients sign and/or read at time of enrollment? I would prefer not to reinvent the wheel - are there enrollment forms for download online that you guys use or could point me to? Having been there and done that I'm sure you guys know what to look for and include much more than I. Thanks - have a great day!

    Here are other threads with tons of useful info in regards to what forms are included in enrollment packets.

    There are several threads with links to sites that have printable forms.

      Thank you Blackcat. I did briefly look through the forum before posting. I didn’t find anything I was in love with. Not knowing exactly what I should have, I’d prefer not to blindly try piecing it all together. Anyone have a good packet of sorts with the whole kit and kaboodle included they could point me to? I was hoping to save my self from a exhaustive google search.

          CALIFORNIA has them on CCL’s website.

          Not contracts, but the required enrollment form that asks medical history and who can pick them up etc.

            Hello. Thanks but I already have all CA mandatory forms. This website https://thedaycarelady.com/form.htm has a vast array of forms to choose from. But they aren't organized by enrollment forms and are pretty generic. I guess it wouldn't hurt to rewrite whatever I needed into my own language. Arg I was hoping to find something a bit more here-ya-go simple. Free wouldn't hurt either while I'm at it. Maybe a different approach... What enrollment forms above and beyond the state provided ones do you gals provide at time of enrollment? If any? Thank you

              I just use the forms provided by the state plus a short contract and one page with my closed dates

              The forms provided by the state cover everything you need to know. The registration and enrollment forms on daycare lady.com are just the same information as licensing’s sheets. But I have a great fill in the blank contract from that site that now has a broken link. If you PM me your email address I can send it.

                Yes I think I finally found what I was looking for! This website http://www.daycareenrollmentforms.com has a well organized home daycare enrollment packet available for download. Not free but worth it in my opinion. Thank you guys for your help and helping narrow down my search. I’m sure I’ll have a hundred more questions down the road . Have a great weekend
