Making A New Form-Need Help With Wording

I'm creating a form to find out what my clients needs are for fall daycare. Specifically, I have 3 children who are at the age when most people go to preschool, and I want to know if I am retaining any of them. (I know one is going off to kindergarten.) I know I could just verbally ask them, but I like forms so I can put it in my files. :: Thanks.
