At what point would you exclude for a diaper rash or yeast infection? And for how long?

At what point would you exclude for a diaper rash or yeast infection? And for how long?

If the diaper rash causes the baby to be in pain or causes discomfort enough to disrupt normal behavior.

I would also exclude AND require a Dr visit if there were open, oozing wounds or if there has been no improvement after 4 days after treating.
