Where Can I Find Communicable Disease Notices?

Hi Everyone!

I am in MN. One of my dc kiddos was diagnosed with Impetigo. When I used to teach in a center, our director would post a notification on classroom doors when an exposure affected a classroom. I loved these notices because they were informative and uniform. In a situation like this with Impetigo- which a lot of parents have never heard of- I would like to post in my family child care- but I cant find the forms! I did a search for the CDC, MN dept. of health, google searches... :confused:

Can anybody tell me where I can find these exposure postings? Thanks for the help!!!!

I inboxed you the file link for ALL the other notices you are referring to. You can find each illness/disease listed in alphabetical order. Each has a Parent Fact sheet you can print for parents and a Technical Fact sheet for your reference. Some offer .pdf files in alternate languages if needed. HTH
