Yikes! I agree--make sure he's hydrated (but it sounds like you know what you're doing, so I'm sure you're keeping tabs on that), and a fear of the toilet could explain refusing to use it, but not the lack of wetting his diaper.

My daughter went back into pull-ups for two weeks after we attended a wedding at a facility with a malfunctioning auto-flush toilet. She was screaming kicking terrified of toilets after that. It's been 8 months and it's still hit or miss if I can get her to sit on a toilet in a public bathroom.

Just video it and show you.

If she can't. You have your answer.

If she does, and the child does what she says, then address that issue.

Bottom line is you can't address the issue if neither you nor mom really know what issue you are trying to fix....kwim?

I had a similar situation a few years back. Turns out mom's idea of telling her/indicating the need to go was completely different than mine.
This was kinda my thinking, that things weren't going as well as she says at home. He does not act at all that he's afraid of the toilet and I use the same type of seat that they do at home. It's the kind that sits up on the toilet. I have stood in the bathroom and also outside the door where he can't see me in case he has a shy bladder and it hasn't made a difference. He did finally go one time today after his nap. He wet his diaper twice today too. His mom has brought M & M's for when he goes on the toilet and I gave him 2 when he went. He really is good at controlling multiple situations mainly with her. He refuses to walk into the house. Wants her to carry him and she does. He throws a fit every single morning when she drops off, but most time recovers within a minute of her leaving. Used to spit and hit her during drop off and pickup but only tried that 1 time with me. I gave him a stern look and stern voice and told him to not even think about it and he didn't. .
