Hello! Completely forgot until now. Does anyone have a quick form for the doctor to fill out stating child is up to date on shots. If not, this is their schedule to get up to date?

I am getting their immunization records tomorrow after the doctors, but I figured I could send this form as well?

Check your states CCR&R web page. Iowa's website has a whole page of provider forms that come from the state, so I know they are in compliance. This form you are seeking is one of the I es the state provides us.

In California, they give us a blue form to have the parents complete at enrollment or ask that we obtain a copy of their yellow immunization card.

To ensure that the records are kept up to date every year during Immunization Month, I send out a mass email to have parents get and turn in a current copy of their child's up-to-date record.
