What is everyone's must ask questions for new families? Does anyone have a "Getting to know the Child" questionnaire?

I am compiling must have questions for my interviews and wondering what everyone else asks....

I have a parent questionnaire that I e-mail families at the time I set up the interview. I ask them to bring the form back filled in to the interview.

During the interview I use a checklist that I created to help me hit all the points I want to make sure I cover. It also helps me stay on track with the interview too.

I email the parents a copy of my Handbook to look over ahead of time and just started sending Interview Questions based on BlackCat's form.

• What are you looking for in a child care provider, program, and environment?
• Has your child been in child care before? Where and for how long? Do you have any concerns about your child attending child care?
• Does your child have any medical, emotional or nutritional issues I should know about?
• What methods of discipline do you use with your child? What seems to work well or not so well?
• How do you feel about your child getting dirty? Minor injuries and risk taking?
• Do you have any questions about the Parent Handbook?

I don't fill it out as we go along, it's more to get the parents thinking ahead of time about the things I want to know about them.
