Does anyone have a form letter they use when a family leaves care? Not a term letter but rather one that says.
Childs last day.
Reason, going to preschool.
Etc.
and that everyone is satisfied w care provided and blah blah blah....
Etc.

What's the purpose of the form? I just have the parents write 2 week notice and state the reason.

I know right?!

The purpose of my form is so that the parents and I are on the same page as to end date, any money due and why they are withdrawing. They sign/date and say they received all their belongings, have filed any grievances if they have any, can give feedback it they wish etc...

Its a great way to "protect" yourself from those families that leave with a smile on their face like you are Mary Poppins only to find out later they turned around and reported you for not giving Jr "special"
