        When parents enroll I have them either get the PM286 (blue) form or submit a current copy of the yellow immunization card. I then input the info in my database and file the copy in the child's physical file. As the child has subsequent immunizations I request an updated copy. Also, once a year during annual "Immunization Week" I send out a bulk email to all the parents to submit an updated record.

          I called licensing and they sent me a huge stack of the blue immunization cards. Once a family enrolls, I request their shot records, record them onto the blue card, and give back their shot records. I make sure to keep them up to date after subsequent Dr visits.
