Taxes: What do You Give Your Clients?

This will be going on my second year of doing home daycare. Last year I did not give out a form. I only had 1 dck. This year I have 5 so I want to give the parents something.

Do you give out a form? if so can I please borrow. I want it to look somewhat professional.

I gave each of my families the w-10 form and I just wrote in the amount they paid me at the top of the form.

if you have minute menu kids, everything is there for you and fills out the entire thing for you too... google minute menu kids. I think they offer a 30 day free trial....
