Request for Exception (Supervision Plan)

I am filing a request for an exception in the state of KS. Our previous surveyor said to be very detailed in the supervision plan. We are required to have a supervision plan in place for licensing. I am assuming they require more detail then that plan. I have started working on mine. I have included how I will supervise during free play, meal time, rest and outside time. Is there anything I am missing? I was also going to include floor plan of house for a visual.

Has anyone ever written one? How detailed do you get?

Include blind spots in your home and yard, explain how your are going to manage these. How will you handle your own bathroom breaks, changing an infant, feeding an infant. What's your plan while your doing your housekeeping that's required during the day and also include you emergency plans, storm, fire, and shelter in place scenarios.

What's your exception for? Here is a copy of my supervision plan, and it's been approved for years in two different counties in KS. I did make a floor plan of my house, but that was for my emergency evacuation plan, not required for supervision plan. I don't watch infants, and didn't include them in the supervision plan, but it should be easy to copy/paste from the 18 months-2.5 years category, then change the sleeping arrangements a bit. Everything else is the same.

Supervision plan:

Ages 0-17 months: Care will not be provided for this age range.
Ages 18 months-2.5 years: Care will be provided Monday through Friday, 7:30 am to 5:30 pm.
Ages 2.5 years-4 years: Care will be provided Monday through Friday, 7:30 am to 5:30 pm.
Ages 5 years and up: Care will be provided Monday through Friday, 7:30 am to 5:30 pm. After school care will also be provided from 3:30 pm to 5:30 pm.

Ages 18 months-2.5 years: Children in this age group will always be supervised in the same room as the provider and never allowed to play in a different room than the provider is in. When downstairs, the children in this age group will stay in the rec room with the provider, or play in the toy room when the provider is in that room. Upstairs for meals, snacks, and craft activities, children in this age group will be seated at the table or escorted to the main floor restroom for hand washing. When outside, children of this age group will always be closely supervised by a provider. There are no areas of the backyard where children cannot be seen. During naptime, all children in daycare will be provided with covered nap mats and placed two feet apart at minimum in the basement rec area. A provider will be in the room with them. Children will be visually checked on at least every 15 minutes and when they wake, a provider will attend to them immediately.

Ages 2.5 years-4 years: Children of this age group will be allowed to be in an adjacent room to the provider, given that they are still within hearing distance of a provider. When downstairs, children are allowed to play in the toy room with the door open if a provider is in the rec area. Upstairs, children will be allowed to wash hands in the hall restroom without being accompanied by a provider. Outside, children of this age group will always be closely supervised by a provider. There are no areas of the backyard where children cannot be seen. During naptime, all children in daycare will be provided with covered nap mats and placed two feet apart at minimum in the basement rec area or in the toy room with the door open. If a child in this age group doesn't need a nap, they will be given a quiet activity to do while the other children sleep. Children will be visually checked on at least every 15 minutes and when they wake, a provider will attend to them immediately.

I was told to write the exception for the 4.5 yr old. The situation is that the family is expecting a third child. The infant would start mid September. I would be over numbers in the 18m-5yr group, until December when child turns 5. But, only in the mornings and non-school days.
