Sample Contract for Summer - Teacher Clients

Does anyone have a contract to share? We have several clients who are teachers and they would like to either not take their child during the summer or considerably decrease the amount of time they attend. Because of the amount of teachers we have we would like to have it in writing on a formal form. Any suggestions on where I could find a sample form for them to sign?
