I give parents a W10 form.

I print it off of Minute Menu Kids.

I use an Excel spreadsheet and document every payment. In January, I pull a copy of each family's itemized payment list and give it to them with their W10. I also import the payments into my profit/loss worksheet, so it helps to have it all in Excel.

Me too. I provide receipts through out the year. If they need an itemized payment list I charge for my time. $27/HR. (I got that figure when I added up my hourly rate at full capacity)
