Parents Night Out

Does anyone know what forms would be required from children not already enrolled for a Parent's Night Out, specifically for Texas?

I'd play it safe and ask your licensing rep. When I've had PNO's, I've only had my enrolled dcks, and my rep just asked for a heads-up anytime I have one.
