I wouldn't write an accident report, but an incident report. If your state doesn't have one you can make our own, google one or maybe someone will share one. I actually need to get one now that I think about it, I have just used pencil and paper but need something more official. On the report you need to put what happened and when (time, date), where (what room?), did you see it happen? Did you only see the effects? If you didn't see it what where you doing and where? How did you become advised of it? An incident report every time can help you if you have to term over this later. Not that you would ever release the report to parents, but you can say it happened on X, X and X dates. Also include on the report what if anything you are doing to prevent the issue. In case licensing ever gets called (because another child gets bit).- Flag

Depends on your state, but probably not. I would, at least, write an incident report for the biter. If you end up needing to term later, you'll have the documentation to show when (and how often) these sort of incidents have occurred. If licensing gets called over an issue, it'll also allow you to prove that you were aware of the situation. I've also had to turn in incident reports to CPS before to help document behavioral changes in cases of suspected abuse/neglect by the parents.
Every center I've ever been at has required parents to sign the incident reports to prove that they were informed of the incident(s). We also provide copies to parents if they want one.
I have an incident report form that I used at a center I used to work for, if anyone needs/wants it. (It is MY form.)- Flag

When someone bites I write two reports.
One report goes to the one who did the biting and the second report goes to the child who was bit. No names are used and parents are required to sign the report and they get a copy - I keep the original in their file.
I wouldn't write a report for your son (since you were there and know how it happened ) but I would write one for the DCB.- Flag
