You do you go about these? Do you have a form you ask parents to fill out? Do you just ask parents to write something down? I need to get some going but am not sure how .

I would just explain to parents (or others you would like to use) that you want to add references to your prospective parent materials and if they would like to write a letter it would be appreciated. I wouldn't' do a form or something of the sort.

So far I have just asked parents if they are willing to be a phone reference and then provided a number but I am going to put a blurb in the next newsletter asking for a small written one as well to put up in my advertising.

I just ask parents if I can use them for a reference and if they say yes I ask if it's OK to give their number and email. They always say yes
