    Immunization Card

    Quick question: The blue immunization card that we are supposed to fill out, do we have to get them from a public health place or can we print one out from online? I live in Cali.

      Double check with your rep, but here in IL if it is printable from DCFs's website we can print it ourselves.

          I get mine for free from the local health dept. We can print them out on blue paper, though.
