Pretty cute and simple layout. It is all online and emails parents a link of the report. I went ahead and did the Free 30 day trial. You do NOT have to enter your credit card information to try it out.
Has anyone else used this? They have an Infant, Toddler, and Preschool form, and they have quite a few really cute themes that you can change at any time. They even have some holiday ones. I emailed the company to see if they had plans to add more holiday/seasonal themes too.
