Looking For a Form To Give DCPs When Issues Arise...

I would like to print a form on brightly colored paper to notify parents when an issue arises that might result in termination of contract. Ie. Late pick ups, late payments, issues with children, and include fees charged if needed.

Like:

Something along those lines? Does anyone have example of one they use?
