    I like the vinegar, conditioner, and a good nit comb (Nitty Gritty is a good one) treatment. Rinse the hair with vinegar, which loosens the nits, then apply a large amount of conditioner (slows the lice so that they can be combed out; also makes it a tangle-free combing, and it's relatively easy to rinse out afterwards) and comb through the hair. I divide the hair (if it's long enough) into sections with hair ties, so that I can comb through systematically and remove everything that needs removing.

    Then vacuum the hell out of everything that can be vacuumed. Bag what can't be either vacuumed or run through the dryer (hottest setting). Vacuum every day for a couple of weeks, and run sheets and pillowcases (and hats/jackets if it's cold out) through the dryer (hottest setting) every day for a few weeks.
