Fever - Gray Area?

I have a 14 month DCB that came in Thursday of last week and had a fever of 101.2. I exclude for anything over 101. I just had him and one other DCB that was napping (separate areas), so I contacted parents and told them it was nap time and they could pick-up following nap time. I got the ole 'It must be teething!" (um... NO, there is no swelling or anywhere in his mouth that looks like a tooth is ready to pop). I have the 24 hour fever rule, but they brought him next day with no fever and said, "He got his immunizations 10 days ago, so was just probably the residual fever from his shots". This could be true, or he could be sick and getting everyone else in the daycare sick? I caved, and probably set myself up for future sickness issues since I let them return. He had no other symptoms and I'm guessing they might have been right on the immunizations thing. I want to be cut and clear and concise (back and white)... but then these gray areas come up. How do you handle those gray areas?

Sustained fever above 101. (100 if under 4 months)

Sustained over 30-45 minutes.

With one or more accompanying symptom (lethargy, nausea, unusual behaviors, etc)

Kids spike random, short acting, fevers from time to time without further illness. It is the invaders that their immune system can't knock out in a short time that we send home.

Parents are being advised that their child does not need to see a pediatrician until after 4-5 days of fever have passed and only if it is still present. They do not want to prescribe antibiotics sooner than that.

Typically when I call for pick up due to fever I make sure to say to the parent, "we will see Timmy on Wednesday!" if it's Monday, etc. This way they don't try to bring back and I don't have to try to decide at the door to turn away.

I send home for fever regardless. They have to be fever free for 24 hours before returning!!!!

I don't have gray areas.

If their temp is 100 or above, they are automatically excluded.

I try to state the date they can come back if I think the parents might "push" the issue.

no grey area here the state reg says 100 and a child shall be moved away from the other children until a parent can pick them up. so if it is 100 they go home no matter what the cause. If there are no other symptoms and the fever is gone with no meds I let them come back

I do this as well (or, really, if they are here and I send home on a Monday then I say I will see them on THURSDAY since I exclude the following 2 school days).

My state says ANYTHING over 100 needs to be excluded regardless of reason (supposed reason, real reason, doesn't matter!). I have stricter rules than my state but that is the bare minimum.

I also exclude at 100. I use the form that you shared with me which is awesome. On it you can check off the proper boxes and dictate what day they get to come back. the parent sign its at pickup and there is no argument about it.

I also exclude for over 100* regardless of why. And if the 14 month old got their MMR vaccine at that visit it could be 7-10+ days out when they start to have a fever. I have a DCG here that just had her MMR and had a little rash so I've read a lot about it in the last 24Hrs. LOL
