Using ASQ-3, Anyone Have a Score Sheet?

I am doing ASQ-3 on DCKs to assess a few that are behind to address with parents, but I only have the PDF files, not the whole program and scoring sheets and such. I'm trying to do 24, 30 & 36 months. Any tips??

You can do the assessment on-line for free and they will generate the socres and send them to you within two weeks:

I found one with a score sheet, are they all the same with a 0 for NY, 5 for S and 10 for Y?
