Form for Waiting List

Does anyone have a form they use for a waiting list? I'm getting calls left and right (Thank you Lord!) and needing to start a waiting list.

I do a wait list, but I enter it into MMK. If you'd like I can whip up a Word doc for you based on the info that MMK requests after dinner.
