    Anyone else have this issue?

    When I Ask parents for a medical form from the dr they only bring back an immunization record.

    I give parents a sheet with all the information that is required to be on the form, Name, date of birth, date of last exam, is child healthy, etc. Then I get back an immunization sheet only.

    I used to give them my state form for the dr to fill out until I had a mom fill it in herself (for her two kids) and left out important information.

    Now I just ask for a print out from the dr office which is usually labeled daycare form or school form and takes only a second to print at most offices

    "Oh Sally! I am so sorry but I need the medical form from you doctor for your child to participate in childcare. All the doctors have the forms. Can you give your doctor a call and request a copy of it? I can't have Suzi in care without it. Thanks"

      I say something similar when things like this have happened ... but I add in THE STATE OF ___ doesn't allow me to have Sally in care UNLESS she has this paper in her file. It is all true, but I make sure I emphasize it so they get how big of a deal it is.

          State mandated here too. It's frustrating. This particular time it for an annual update. I gave dad my list of required info and even read it to him, he brings back immunizations ugh!

          I didn't realize right away because it was folded up and he left. I texted him to ask about it, he says oh I forgot. I'll get it tomorrow.

          I don't need it until the middle of june since it's an annual and that was his start date last year. Glad I asked for it now and didn't wait until we got closer!

            Irequire the state forms to be filled out but allow computer copy of immunizations instead of writing those on the forms .
