    Ok, so my website is finally up.. www.JimNaisium.com It is still a work in progress, and prolly will be changing from time to time. If there is anything obvious that I missed, don't be afraid to tell me.

    I personally would make the contract and the registration paperwork seperate from the policies and both you and the parents/guardians sign and date contract (contract is suppose to be for the money issues). It doesn't come across as a contract to me in the form it's in at moment. I would also list illness symptoms excluded for because right now it's too vague.

    Also where is your emergency form, medicine permission, photo permission etc

      Yea.. I was thinking that the contract should be split out to have a seperate handbook.. my emergency form is a blue card that I have a stack of here. In NY we are required to have one for each child and it has a picture of the child, allergies, contact info, medicines they are on, etc. I am not actually allowed to give medicine (even with the mat training) unless the parent and a doctor tell me to in writing, in which case those documents themselves get filed and that's my form. I didn't even think of a photo permission form, but perhaps not a bad idea to have one.
