What do you include on your booboo reports?

I use the states although I am not required to unless they need medical care. I do it for any mark...::

It includes HOW they got the injury, WHERE the injury is, and a DESCRIPTION of the injury (I do rough size, color, bleeding/not bleeding, etc.). I also include what was done for the injury (ice, bandaid, cleaned, etc.).

Our form has on the top, Child's name, date, time, child's classroom (for center based), where the incident happened (in the classroom, outside, etc)

Then the main part is description of incident, description of injury, first aid given

At the bottom is a place to mark if the child's parents were called, a line for any notes, and a place for the name of the person (or people) who took care of the injury.

I use my state's form also. Ours can also be used for illnesses, which is nice. I love that it has a section for future documentation about the incident. Here's a link to our form just so you can see all that it includes.
