Withdrawal Form?

I have a dcg withdrawing today and I saw a great letter here when searching and now cant find it!
I think it was BC's....

It included spaces for whether they would return, did they collect all belongings, etc.

if anyone knows which one im talking about, I could use some help finding it or has a similar one I can borrow.

I think I found what you were looking for, but because I am computer illiterate, here's the route I took:

Advanced search "withdraw form"
"Termination/Withdrawal Forms" thread
from that thread click on the "withdraw form" tag at the bottom
"A Form For Families Leaving" thread
Blackcat's withdrawal form is in that thread.

I don't mean to step on BC's toes, but it seemed like you were looking for something quickly....although this was not the quickest route to take

Sorry ladies.... I'm glad you found it.

The search function can be hard to navigate sometimes.

Hopefully the form will still work, even though the family has already left.

You could just let them know you need it to close out their file, maybe they'll be nice enough to fill it in and return it.
