    state pay contract

    I have my first state pay parent in over 20 years.

    I need a contract form stating that the parent will pay whatever the state doesn't. (ex: parent was on spring break last week and only came 2 days, state won't pay for the full week, only 2 days.)

    Anybody have one?

    My handbook has a section on state assistance. Here is what it says.... You are welcome to use any or all of it and tweak it to fit your needs.

    State Subsidy Program

    ABC Child Care accepts payment via Child Care Assistance Program (CCAP) college grants and other funding sources as payment for services.

    Proof of qualification and/or acceptance for these programs is required before care will be provided..

    If acceptance is pending, parents must pay all fees until first payment by qualifying agency is made, at which time the parent will be reimbursed for any services covered by participating program.

    The child care assistance program (CCAP) does not cover the entire rate billed. Parents are responsible for any amounts the state does not cover.. This is referred to as "parent fees"

    Most families using CCAP will be assessed a co-pay. This amount is due BEFORE the state will pay for any additional services provided.

    Both parent fees and co-pays are due PRIOR to the upcoming 2 week service period. Any fees not paid will have the appropriate late fee added and no services will be provided until balance is paid in full.
