Help with wording please

I'm possibly terming a family, but I'm giving them an option to keep their spot for more money. The issue is that they pick up at random times without notice despite many convos and promises to do better and so sorrys, and I'm with an agency that doesn't allow me to charge late pickup fees. I can charge them a higher rate if they agree to it. Otherwise, they can hit the road.
I'm not good at putting my thoughts into words, so here is what I'm working with.

I am giving two weeks notice for change.
If you wish to keep Princess's spot with me, continuing with hours being used, with pickup window closing at x:xx pm, you will be billed at the Extended Day rate.
If you chose not to use this option, final day of care will be xxxx 2014.


I'm going to include a copy of the hours they've been using since the new year.

I know it's sloppy, I need some help straightening it out please.

Something like this maybe? Just give them the options available and let them pick. Set a timeline as to when they need to let you know by though...
