    Co-Payment Contract

    For those that take state pay and parent is responsible for a copayment do you have a separate form for them to fill out besides your handbook/contract?

    Anyone care to share what they have? I have my first state pay client and not sure what I should include.

    Well, I only have one now. Since the state changed to attendance based, I charge her slightly more than their co-pay, and the extra more or less covers her absent days. That way, she knows what she has to pay every week. Otherwise, if she misses work due to illness, she also has to pay more to me, which she can't,

    According to the state, her copay would be $21.25 per week (based on her hourly rate and our county max). They have her authorized for 38 hours (but of course, the cap at 35, so that's stupid).

    I have her pay $30.00 per week, every week, regardless of attendance. So, every week $9.75 goes towards vacations, illness, etc. That's $507 per year. I just calculated out what she paid me last year (state and out of pocket), and it comes out almost exactly right). It's enough, really, to cover almost 4 weeks of absences! But, they add up quickly.

    So, her contract is the same as everyone else's, except it shows her co-pay instead of full tuition, and is subject to change if her subsidy funding changes significantly.

      I use the same contract for all parents but have a space for them to indicate where the funding is coming from.

      For example, they can choose to pay weekly, bi-weekly, other or CCAP (which is our assistance program).

      I have a line where parents can write in their co-pay amount or rate amount.


      Same contract for everyone but each family lists their individual payment method and their individual payment schedule.
