Form for New DCK

Does anyone have a type of "All About your Child" form they wouldn't mind sharing? I don't like the one I have and I need to get my paperwork to a new family starting next week. Just something that lets parents fill in things like home/sleeping schedule, eating routines, allergies, preferences, ect.
