I am putting together a portfolio for potential clients and future jobs down the road. I want to give current parents an optional recommendation form for them to fill out so I can include it in my portfolio. Any suggestions on the questions or format?

Future jobs down the road for you? If so, I would think that would make parents nervous that you are only doing this temporarily and are planning to stop providing care soon... kwim?

As for future jobs, it is the reality I live in as I know you saw from my previous post. We all have DEROS (date when we should be leaving). Mine is in two years, but now with budget cuts they are putting every active duty member in a board to determine if they stay in or not. However, parents have found it nice to find a provider who wants a future in child development rather than a stay at home mom who is just trying to keeo working. I could go into the economics of being a military spouse......but it is rather boring, haha.- Flag

Right now it is for future parents. I want them to open a book with my curriculum, observations and what other parents have to say. Plus photos of kids activities. Obviously I wouldn't include children without permission from the parents.

What I would do in your shoes then is outright ask parents to write something up for you.
Maybe tell them since a majority of care is done on a part time or temporary basis due to being a military provider that you would love to show all potential future clients what goes on within your program.
Ask if they would be willing to write up a few paragraphs that reflect what they love about you and your daycare, both professionally and personally.
I've asked parents in the past to write me letters of recommendation so I could use them on my website and for future clients. Everyone I asked, was more than willing.
