Parent medication form

I know I have seen several people on this board mention that if a parent either gives meds prior to drop off or comes to administer them during the day that they have a form the parents fill out to say what they gave them, why, how much, etc. Anyone willing to share their form?

Here is mine. It's pretty simple.

Thanks BC. I think I will modify it some. I definitely want a space that says why they gave it to them, whether it is the first time they have had the med, and possible side effects.
