I need your help with some ideas or examples for program evaluation survey's I need to give out for parents to fill out regarding the program.

I do a rating system 1 to 10.
Please rate satisfaction with:
Cleanliness
Curriculum
Arts and Craft
Communication with provide
Clarity of policies
Overall satisfaction

I also ask them to name things they like about the program, things they would like to see improved.
