When do you fill out an accident report? A little girl just fell off of 1 step at my place. Just a scratch in her elbow but says her leg hurts and she won't put any weight on it. No swelling, full range on motion, but says it hurts. She's 2yo. Her mom just came to get her to take home, dr said give her Motrin and see what happens. Do I fill out a report? Do I wait to see if she goes to the dr and see what he says first? Help!

I would at least document it just in case (keep it in her file). You never know, like the recent thread about the DCK at the kiddie gym and the kid wound up with a hairline fracture in his foot. I would also try to call and ask your licensor as soon as possible.
The pain could just be a sore or cramped muscle if she locked her knees to try to break her fall.- Flag

Report it if she needs to see a doctor. You would need to report it via phone or fax to your licensing agency within 24 hours of medical care. Here is the form:

Yes this.. Even if the child does not have to go to the doc because of it, make a full report for your file and have parent sign it.
If mom calls you later and says we ended up having to take her to the doc then you need to fill out the incident form and report it as soon as possible.- Flag

Reporting requirements require licensees to report to CCLD by telephone or fax within 24-hours any events listed in Health and Safety Code (HSC) Section 1597.467(b)(1) and California Code of Regulations (CCR) Section 102416.2(b) for Family Child Care Homes (FCCH), and any events listed in CCR Section 101212(d)(1), for Child Care Centers (CCC) that occurred during their operating hours. In addition, some circumstances may require the Licensee to inform other local authorities, such as the police/sheriff, local health department, or county child protective services of the incident.
A written report shall consist of the following:
•Child’s name, age, sex, and date of admission
•Date and nature of the event
•Attending physician’s name and findings and treatment, if any
•Disposition of the case
Licensees are to document the events described in HSC Section 1597.467(b)(1) and CCR Section 101212(d)(1), on the Unusual Incident /Injury Report (LIC 624 for Child Care Centers and the LIC 624B for Family Child Care Homes). The Unusual Incident/Injury Report must be submitted to CCLD within seven days of the event. Upon receipt of the report, the Licensing Program Analyst must determine whether the incident warrants an immediate case management visit to the facility or a follow-up. Although the Unusual Incident/Injury Report is not recorded as a complaint, the same steps may be applied to assist in the investigation, such as interviewing the victim and witnesses and collecting documents. In cases where another investigative agency has conducted an investigation prior to CCLD, a copy of their report may be sufficient and additional interviews or a facility visit may not be necessary.
Lastly, licensees are to notify the parent/authorized representative as soon as possible when there is reasonable cause that a child in care has suffered any injury or has been subject to an act of violence under the care of the licensee. As a result of the immediate notification requirement, a parent conference may convene to discuss the nature of the event.- Flag
