    Change in Schedule Request Form

    Okay ladies ... I'm tired of having to give out new contracts to clients every time they change their schedule. I don't mind that my clients change their schedules, I mind having to print out a new contract, fill it out, calculate their new rates and have them sign it etc. It just seems wasteful.

    I'm thinking about having them fill out a form that I can give them where they request to have their schedules changed and we simply fill in their new scheduled hours and new rate. Then instead of printing off another 7 pages I can just print one page out, fill it out, they sign it, I sign it and then I just attach it to their contract. Anyone have something similar to this that I can get ideas from?

    My form for contract time is just one single page... If it changes I print that one page (actually have extra printed already ) and we fill in the new hours and the new rate and both sign. Takes 2 minutes
