Form Letter To Address Behavior

One of my DCKs doctor has asked for the teacher and child care provider to write a letter regarding dcks behavioral and social developmental issues.
I've never had to do anything like this before. I've had ongoing discussions with the parents over the years and I was the one who pushed for the evaluation. But how should I word it, or is there a specific form I should use, like a checklist?

I have my sons preschool fill out a form that the dr provided me with and they mailed it to him so I wouldn't see their answers. I don't know if I would feel comfortable writing a random letter and handing it to the parents to read.

I would ask for a form and a preaddressed envelope already stamped for me to do that.
