    Strep? Fever? Need Advice

    Three weeks ago, quite a few kids in my DC came down with strep, as did I. One of my DCDs also got quite sick. When he got sick, I insisted that their 'symptomless' child get tested. She had been running a low grade fever for a week or more, which they insisted was 'teething'. She tested positive for strep, which was not a surprise to me and she was treated. Anyway, everyone got treated that tested positive and I thought we were good to go.

    So yesterday another family of two calls me as their two kids tested positive again. They are really frustrated, but did not blame anyone. I called all my families and asked them to get tested. All but one family got tested, and they said they were going to do it at her check up appointment on Wednesday. I told them that this was not ideal, because if it did come back positive...everyone that was healthy was exposed again on Monday and Tuesday. They came today and stuck to their guns about not getting tested until Wednesday. Until...

    The little girl woke up from nap feeling warm. I took her temp and it was 100.4 then 100.9, 30 minutes later. I called the parents and said she would need to be picked up and cannot return without a negative strep report from the doctor. Regardless of what we learned last time around, they insist this fever is from 'teething'. I told them that regardless, the temp was too high too be in daycare tomorrow and they would be wise to get her strep checked TODAY, so she can get put on antibiotics and come back to daycare sooner if she is positive. I can tell they are angry with me, which is so hard on me when I am trying to do the right thing by everyone.

    What would the rest of you do with this scenario when it comes up? What is your cut off temperature where they can't come the next day?

    Would you make your other clients all get tested for strep if you keep having a reoccurance?

    These are really nice people, and I understand how inconvenient it can be, but I can't let a child be here if I suspect they are sick.

    I would exclude based off the symptoms they have. I wouldn't require a strep test, even though I'd love to. Most of the time the doctors here don't even test for strep. If they suspect it, they treat for it, but generally don't waste the time and money on the test.

    I would make sure they don't come back until they are symptoms free for atleast 24 hours. I have asked for a doctor's note on occassion, but nothing more than that.

      Did the family who tested positive again get new toothbrushes after they got over strep the first time? Just a thought as I had this happen once with a family. Kept testing positive over and over. Finally asked them about the toothbrushes and they were using the same ones. They got new ones and problem solved.
