Sign In/Out Form

I want to change my sign in sheet and I have an idea of what I want it to look like but I don't know if I want to go to the trouble of trying to make one on word. Did any of you find yours online? Did you make them yourself?

I think I want it to be weekly with a column with each child's name and then a row of Monday through Friday in and out slots for each child. I also want to have a spot for caregiver in and out( my licensing worker said I needed that) for each day.

Does anybody have a link to something like this?

I use these, and for licensing they have worked well. Our licensing isn't very specific about signing in and out, and what info we need. I use the one with a signature line because I figure the extra documentation can't hurt. I also like the weekly one, because it is less paper work to keep track of and I only have to change it every Monday.
