    Interview Questionnaire...What To Add?

    I have contact info for parents, place of employment, who is primary contact, children's names, ages, special instructions, how they heard about me, their desire to be placed on waiting list, and a spot for them to write out any questions/thoughts/concerns.

    Any thing else I should put on? I'm doing a questionnaire as an easy way to keep records of parents and kids...so I can call them when I have openings, if they call back looking for openings (so I can look at it, and remember why they didn't get a spot).

    Anyone else do this? And anything else important that I'm missing?

    Is child currently attending a daycare or preschool?
    Are they looking for part time:full time

    I need coffee and ill think of more. Lol.

      I don't really use email to communicate with parents, so I didn't add that. I did have hours/days care is needed, but forgot to tell that in my op.

      However, I did add currently enrolled based on your suggestion, plus if this is different hours than before. I just termed a child that was having behavioral issues, and mom told me after working with this child for 4 months that he had never been in daycare full time before, it was always part time. Wouldn't have changed anything really, but perhaps I should have been told that!!
