Need Help Making Parent Survey

Part of the new requirements for the state are that i survey my parents annually. Does anyone have some ideas for me or even basic questions i would ask?

Also some kind of written goals

Is the facitlity a clean environment.

my goals are to expand my science curriculum

My goal it to create a natural playground

My goal is to do more hands on art activites that are process oriented and less precut crafts that are product oriented.

My goal is to do my observations and make changes according to those observation

This is my annual survey.. I offer a 10% discount to parents that turn it in and also include a section for leaving a testimonial to put on my website with their concent.
