    What Is Your Policy Regarding Medical Release Forms?

    Do you have one form that all parents sign?

    Do they sign something additional if there is a specific medical issue?

    Do you ever administer medications while child is in your care?

    I would like to see some other options for medical release forms if anyone would like to share.

    I NEVER administer ANY meds with the exception of prescribed medications and ONLY if they are in the original bottle and have the child's name on it as well as directions for use.

    I allow parents to give Tylenol/Advil before drop off for teething or discomfort as long as it isn't related to a contagious illness but they have to sign off on a form and state why and when meds were given as well as who gave them.

    What do you mean by medical release forms? I have a form I have parents bring to the doctor so that I know what their diagnosis is and what the recommended exclusion period is and all...do you mean like that? or is I have a kid with a specific medical condition that is on-going?

      I don't use a specific form but the worst I've had was allergies and a child who had to be careful because she had several surgeries on her skull. In both cases I had the parents write up a synopsis of the child's allergy or ailment and attached it to the release so if there was an emergency regarding either of those things the paramedics would be aware of the history.
      Do you ever administer medications while child is in your care? No, it's too much of a liabilty. If a child needs tylenol or another OTC they probably shouldn't be in childcare. If they have an antibiotic or other prescribed medication the parents can arrange it so the child takes it at home. I even do this with 3x a day meds. If they give it to the child at 7:30 am/5:30 pm/11:30 pm they can still do 3x a day and in my experience 3x a day meds only last a few weeks at the most so it's doable. I have so many comings and goings a day not to mention diaper changes, hand washes etc. the last thing I need is one more thing that I HAVE to do.

        We have to use forms provided by the state of Kansas for all of the above.
