Before/After Care Contract

So I have my regular contract and I then have a separate 1 page part time contract that just outline/re-specifies drop off time, pick up time, rate, days, rate per hour if they're here longer, etc.
I'm wondering if anyone has something like this for before/after school? Basically pick up/drop of, rate, rate on days there aren't school, etc.

Also do you guys contract the rates for before/after school - like its always a set $ a week or do you just charge them for when they are there?

Mine is all in one contract. I then have a page that the parent fills out the schedule for the child, so they would put the before/after school time and then at the bottom we write any Days Off From School.

I charge a monthly rate for Before or After school and Before and After School.
