Permission to swim / permission to transport

I'm not having luck with searches on this. I'm looking for a permission to transport form (not just for emergency but for field trips, etc.) as well as a permission to swim.
Preferably with some sort of liability waiver attached in case of accident.

Does anyone have either of these that they can share, or know where I can/should get one drawn up?
Has anyone used the rocket lawyer web site?
