    Daycare References

    I was wondering if any providers have their clients complete a reference form yearly, or when leaving care. If so, what are questions to ask on the form. I have a generic form the the state used to use, but it is almost 20 years old. Looking for suggestions! Thanks!!

      I did that but it was for a child development training program that I did for the stipend project. It asked open ended questions like "What do you like about the daycare", "describe the relationship that the child has with the child care provider" etc. It had more but they were questions like that.

      I only did it for the stipend project though, other than that I just keep birthday cards and such that they write nice things in and use that. I also have a testimonial page on my website that parents can write a note in. I always ask the families to do it but more often than not it's the parents that leave that always give me a thank you letter or write a testimonial. I think it sounds better when it's from the heart instead of written on a form. I can show potential clients the real letters and cards and it looks better than me showing them the filled out forms (like a worksheet).
