Outdoor Permission Slip

Does anyone have a permission slip that covers everything outdoors? scooters, jungle gym, trampoline, water play... that type of thing?

I would like to find a way to word it so that I am covering everything and asking that the parents supply helmets

None of those are allowed here - water play if less than a few inches deep and dumped everyday or sprinklers. Play set is ok. But nothing with helmets required and no trampolines ever per regs.
