I think as far as the food program goes just have them write you a note saying what type of food they want you to give him and have them provide it then just don't count his meals for the food program. If the parents are giving him junk food and that's why he is having such a hard time then a good way to get the parents to stop is to make them responsible for providing all of the child's meals at DC. Let them know that you are concerned about the child's eating habits and that you will start keeping a log for yourself of what and how much he eats. Maybe it'll worry them enough to stop the junk food at home and get some help.

. If you're concerned about his weight bring out a scale and weigh him and write down his weight in your log. That way you can keep track of wether or not he's losing weight. I'd be concerned about his health and emotional state from the junk. Usually I wouldn't be worried but only if a child was eating something at home (other than junk food).
